Your ID Card

A restor3d patient ID card makes travel more convenient* and makes it easier to let others know that you have a personalized restor3d implant. The card will list your name, surgery date, surgeon’s name, and surgeon’s phone number with a representative image of the type of implant you received.

* A patient ID card is not required for air travel. You will be allowed through the security scanner without presenting this card.
